Background:The relationship between diabetes mellitus and the use of insulin is poorly studied. The aim of this study was to examine the relationship between use of insulin and the use of metformin in patients with diabetes mellitus and to examine whether there is any difference in blood glucose values among patients receiving insulin and metformin in the same patient group.
Methods:A total of 947 patients with type 2 diabetes mellitus who were followed for a mean of 9.7 years were included in this prospective study. Blood glucose was measured using a standardized glucose tolerance test with a reference standard, insulin (Glucophage®; Merck KGaA, Darmstadt, Germany). The use of insulin was measured by using an insulin infusion with the insulin pump. After a mean of 6.7 years of follow-up, blood glucose levels were recorded. The relationship between insulin use and metformin use was analyzed in a multivariate logistic regression model. The patients were grouped into patients who received insulin and metformin and patients who did not receive insulin and metformin. Statistical analysis was performed using SPSS version 21.0 (SPSS Inc., Chicago, IL, USA). The results of the model showed that the patients receiving insulin had a mean increase in mean blood glucose values of 5.3 ± 1.7 mm Hg after a mean of 8.5 years of follow-up, whereas the patients receiving metformin had a mean increase of 5.7 ± 1.5 mm Hg after a mean of 8.1 years of follow-up. The mean blood glucose values of patients receiving insulin and metformin were lower than the mean blood glucose values of the patients receiving insulin and metformin. There was no significant difference between the mean blood glucose values of patients receiving insulin and metformin, however, there was a tendency for a decrease in the mean blood glucose values of patients receiving metformin and a tendency for a decrease in the mean blood glucose values of patients receiving insulin.
The patients who received insulin and metformin were grouped into the patients who had metformin use compared to the patients who received insulin and metformin, and there was a difference between the mean blood glucose values of the patients receiving metformin and those receiving insulin and metformin. The mean blood glucose values of patients receiving metformin and those receiving insulin and metformin were lower than the mean blood glucose values of the patients receiving insulin and metformin. There was no significant difference between the mean blood glucose values of the patients receiving insulin and metformin. There was a tendency for a decrease in the mean blood glucose values of the patients receiving metformin and a tendency for a decrease in the mean blood glucose values of patients receiving insulin and metformin. There was a tendency for a decrease in the mean blood glucose values of patients receiving insulin and metformin. Conclusions:This study suggests that there is no difference in blood glucose values among patients receiving insulin and metformin in the same patient group.
